The Following puppy is guranteed by te Breeder, Cindy Hubbs of H&H Chinese Cresteds to be healthy and free of genetic
defect. This contract is set forth to outline the specific responsibility of the Breeder toward the health of the puppy described
herein:

The Breeder gurantees the above described puppy to be healthy and free of communicable diseases and geentic defect. To
activate this guarantee, the puppy buyer must have this puppy examined by his/ her personal Veterinarian within three (3)
days of purchase and physical posession. Failure to have the puppy examined within the above mentioned time frame voids the
contract in it's entirety and removes the Breeder from any further responsibility towards the health of said puppy. (A copy
of the dated receipt and vet report must be maintained and accompany this written gurantee for any claim to be considered
valid)
This puppy has been vaccinated on the following date(s) and will require ___ additional vaccinations at three week intervals
with a booster given at one year and then every year thereafter to achieve and maintain immunity to common communicable
canine diseases. Failure to properly vaccinate this puppy can result in severe illness and death from preventable diseases
and constitutes gross negligence on he part of the owner. You must complete and maintain your puppy/ dog's vaccinations!

This puppy is guranteed to be free of genetic diseases and defect to include organ defect, Patellar Luxation, Legg-Calve-Perthes
Disease, PRA and congenital deafness. If your dog should be diagnosed with a listed genetic/ congenital defect within
12 months of purchase, the Breeder wil provide a replacement pup of the same sex and quality from the next available litter
or the litter or the Buyers choice. A repalcement puppy wil only be granted after the affected dog has been spayed/ neutered,
if the dog is not already altered. The Breeder is not reponsible for any costs associated with diagnosis of genetic or congenital
disease or defect. The Breeder si not responsible for shipping costs, if any.
The Breeder reserves the right to have any puppy examined by a second Veterinarian of the Breeders choice at the expence
of the puppy owner for the purpose of a second oppinion regarding any claims to this health gurantee, In the case of Patellar
Luxation and/ or Legg-Calve-Perthes Disease, the claim must be accompanied by a statement from OFA confirming that the dog
does have Luxating Patellas and/ or femoral head degeneration consistant with Leg-Calve-Perthes Diesease. In the case of PRA,
the claim must be accompanied by a statement from a Certified Veterinary Opthamologist with a CERF endorsement stating that
the animal has been diagnosed with PRA.
All claims to this Health Guarantee must be accompanied by a statement from your Veterinarian on the Veterinary Clinics
letterhead, along with a copy of of the receipt from your original "new puppy" exam to be considered valid. This guarantee
is not transferable and is valid only for the original Buyer.
